American Cancer Society – Transportation Assistance
Road to Recovery is an American Cancer Society service program that provides transportation for cancer
patients to their treatments and home again. Transportation is provided according to the needs and

CancerCare Assist - Financial Assistance
In order to qualify, you need to provide CancerCare with information about your income, savings and
expenses, and in some cases provide invoices for the cost of service. Your healthcare professional must

Partnership for Prescription Assistance – Prescription Assistance
The Partnership for Prescription Assistance brings together America's pharmaceutical companies,
doctors, other health care providers, patient advocacy organizations and community groups to help
qualifying patients who lack prescription coverage get the medicines they need through the public or
private program that's right for them. Many will get them free or nearly free. Among the organizations
collaborating on this program are the American Academy of Family Physicians, the American
Autoimmune Related Diseases Association, the Lupus Foundation of America, the NAACP, the National
Alliance for Hispanic Health and the National Medical Association. To access the Partnership for

GlaxoSmithKline (GSK) – Drug Assistance
GSK Access - is a program sponsored by GlaxoSmithKline that provides GlaxoSmithKline prescription
medications at no cost to Medicare Part D Prescription Drug Plan enrollees who meet the eligibility
requirements. Eligibility is based on annual household income and proof that the applicant has spent
$600 or more for prescription medicines for the year. A completed application along with income
documentation and proof of prescription expenses must be mailed to GSK Access for processing.
Applicants will be notified if they qualify for the program and, if approved, a pharmacy card will be mailed
to the applicant that may be used at any retail pharmacy to pick up GlaxoSmithKline medicines at no cost.
Drugs received from this program do not count toward True Out-of-Pocket Spending (TrOOP).

The Patient Advocate Foundation – Co-Pay Assistance
The Patient Advocate Foundation (PAF) Co-Pay Relief Program (CPR) currently provides direct financial
support to insured patients, including Medicare Part D beneficiaries, who must financially and medically
qualify to access pharmaceutical co-payment assistance. The program offers personal service to all
patients through the use of call counselors; personally guiding patients through the enrolment process.

Linda Creed Breast Cancer Foundation – Non-Medical Financial Assistance
The Emergency Assistance Fund of Linda Creed Breast Cancer Foundation fulfills a crucial need to
provide non-medical financial assistance to women facing day-to-day hardships while dealing with breast
cancer treatment, its effects and other needs as determined by the Emergency Assistance Fund Review
Committee. If you are currently receiving treatment and are in need of emergency financial assistance please call the

The Pink Ribbon Riders mission is to promote breast cancer awareness in the motor sports industry for both men and women, while raising funds for those in need. In combining these two goals for our organization, we hope to bring people in the community closer as this disease not only affects those diagnosed but also those who love them. The Pink Ribbon Riders is a small volunteer based 501(c)(3) non profit organization that raises funds through motor sports related events. Funds raised are distributed through an assistance program that is made available to both men and women breast cancer patients. The Pink Ribbon Riders is a grass roots effort.
